Job Summary:
Under the supervision of the Manager of Rehab and the Assistant Rehab Manager, the Physical Therapist evaluates and treats patients, provides daily documentation, and coordinates with other staff.
+ Makes home visits in order to evaluate the patient type and degree of disability, potential for improved function, and special problems affecting the rehabilitation program.
+ Completes clinical notes; submits visit documentation, initial evaluations, physician's orders referrals, re-certifications, resumptions of care, progress reports and discharges in a timely manner and in accordance with agency policies and procedures.
+ Participates in ongoing interdisciplinary assessment of assigned patients.
+ Develops and evaluates the plan of care in partnership with the patient, caregivers and any patient identified representatives.
+ Provides patient care in accordance with physician orders and the plan of care.
+ Plans, implements, and updates a treatment program and coordinates these plans with other Agency services to help provide efficient and total care.
+ Confers with physicians regarding initial treatment plan, subsequent changes in plans, orders for specific modalities and exercises, prescriptions for orthopedic appliances and other special equipment. Plans for eventual referral to outpatient facilities when the patient is no longer homebound.
+ Re-evaluates functional ability and progress of the patient. Revises goals/plans and documents these changes timely.
+ Delegates and supervises the patient care of assistants, support staff, students, and when necessary Home Health Aides per procedure.
+ Provides patient, caregiver, and family counseling and patient and caregiver education; provides the patient/family and others who inquire with appropriate information through use of educational material and/or directing them to the proper source for their information. .
+ Attends team and staff meetings as required.
+ Participates in the agency's Quality Assessment Performance Improvement Program.
+ Participates in agency-sponsored In-service Training.
+ Participates when necessary as a member of the hospice interdisciplinary team.
+ Completes mandatory education and training in order to maintain organization and department specific competencies and requirement. Maintains applicable certification/licensure and required CPR certification.
+ Follows hospital/system and department policies and procedures with specific attention to attendance and punctuality, dress code, and display of ID badge, and safety.
+ Other duties as assigned
Education:
+ Bachelors degree in Physical Therapy.
Certifications:
+ Physical Therapy current license to practice in Ohio.Basic Life Support (BLS) Certification.
Complexity of Work:
+ Good communication and interpersonal skills required.
+ Within scope of job, requires critical thinking, decisive judgment and the ability to work with minimal supervision.
+ Must be able to work in a stressful environment.
+ Must be able to read, write and communicate with patients.
Work Experience:
+ One year PT experience in acute or Rehab setting preferred.
Physical Requirements:
+ Must be able to travel to patient's home.
+ Must be able to climb stairs, pull, push and facilitate movement of the patient.
+ Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move more than 100 pounds.
+ Must be able to transport laptop computer and/or necessary medical/patient equipment and supplies.

+ Follows standard precautions using personal protective equipment as required
